Jerry Cooks, 54, of Mediapolis, passed away on Monday, September 14, 2020, at the Mediapolis Care Facility. No visitation or services will be held at this time. Jerry was born on December 12, 1965 in Chicago, Illinois, the son of Leon Cooks, Jr. and Virgie Perry. Jerry loved to listen to music and take walks. His favorite rapper was Nelly.

Jerry will be missed by his father, Leon Cooks, Jr. of Chicago; siblings, Terry Cooks, Linda Dyson, Patrick Cooks, Jermaine Perry, Lashaun Perry, Rodney Cooks, Yulanda Long, Dionne Cooks and Melanee Cooks and a host of nieces, nephews, great nieces and great nephews.

Jerry was preceded in death by his mother, Virgie Perry; sister, Janette Cooks and brother, Jose Perry.
